A script is very useful with preparing a difficult phone call. Here is a small example for a phone call after a rejected job application. Perhaps it is somewhat outdated. But it illustrates how you can prepare questions.

First you make sure that the person is not totally distracted with something more important for him.
Then it helps to hear the reason for the rejection. Unfortunately reasons given may hurt you and may be inaccurate (in your opinion). You however need to hear and accept the answer before you step to the other two questions.
